Confidential handling of medical information

The content of the information in the clinical record is confidential, in accordance with the country's current regulations, and we ensure compliance with this condition through the application of protocols, regulations and policies in the hospital.

Medical records

It is a dependency of the Medical Direction specialized in the management of the information system of the health services within the Clínica Bíblica, where our responsibilities stand out:

  • Custody of the clinical information of our patients (clinical file).
  • Elaboration of health statistics.
  • Supervise the quality of the clinical file.
  • Maintain the availability of the clinical file to guarantee the continuity of care and evolution of our patients.

Clinical Record Committee

It is a local technical body in charge of organizing, in compliance with national and institutional regulations, the regulations on procedures, use, custody, conservation and conformation of clinical records.

It is formed for each of the following services: Medical Management, Nursing Management, Legal Counsel, Medical Records, Admission and Hospital Quality Unit.

Confidentiality

Each patient who receives care in our institution has a clinical record, in order to ensure the integration and continuity of care and as the record is a legal medical document, we seek at all times to maintain vigilance and quality control of clinical information and the protection of confidentiality that it provides to users.

The content of the information in the clinical record is confidential, in accordance with the current regulations of the country, we ensure compliance with this condition with the application of protocols, regulations and policies in the hospital.

In this sense, in the Clínica Bíblica Hospital and by disposition of articles 189, 190 and 191 of the General Law of Hospitals of the Republic of Costa Rica, it is strictly forbidden to provide clinical records to third parties, except with written authorization from the patient, duly authenticated and complying with the requirements of the law.

Certificates of birth, death, epicrisis and others

Epicrisis

Epicrisis

  1. An epicrisis is a clinical summary of a patient's inpatient or outpatient visit. This summary may include:
    • Reason for Admission or Consultation,
    • Admission and discharge diagnosis,
    • If any surgical procedure was performed,
    • Medical Treatments, and
    • Discharge Condition.
  2. To process an Epicrisis, the person can:
    • Call directly to our call center 2522-100 to request the procedure.
    • Request it by e-mail: registrosmedicos@clinicabiblica.com,(when doing it by e-mail, you will be asked to send a copy of the patient's identification document).
    • If you come in person, please bring the patient's identification document and go to the Clínica Bíblica South Lobby Reception.
    • Complete the Medical Document Request Form (FO.RM-012), specify the day of consultation and admission.
    • The process takes 8 working days and you will be informed by phone to pick up the document.
  3. In case it is requested or withdrawn by a third party, identification documents and written authorization of the patient, duly authenticated by a lawyer and complying with the requirements established by law in the General Hospital Regulations of the Republic of Costa Rica in its articles #189, #190, #191, must be provided.

Certificate of birth declaration

Certificate of birth declaration

The birth declaration certificate is the official document that declares the birth of an individual, which is signed by the interested parties before an Assistant Registrar of the Civil Registry.

To request a Declaration of Birth certificate:

  • The parent must appear to sign the declaration of birth within eight days of the birth.
  • Carry an identification document of the father or mother (identity card, passport or residence card).
  • Verify that all the information in the vital event is correct.
  • If the mother is single, married (when the birth occurs within 365 days after the civil event), is in a free union, is judicially separated or divorced, and the child was born after 300 days of the judicial separation or divorce, both must present themselves to sign the declaration of birth.
  • In cases where the parents were minors at the time of birth, they must be accompanied by an adult with a degree of consanguinity.
  • If the parents are foreigners or were married abroad, the father and mother must appear to file the declaration of birth at the respective office of the Hospital.
  • When the birth occurs after the 365-day period following the marriage, one of the parents must sign the birth declaration certificate.
  • The Medical Records department sends the certificates previously signed by the parents to the Civil Registry on a weekly basis for the due registration process.

Proof of birth

The birth certificate is issued to certify the birth of an individual. The certificates are requested for different purposes, by different institutions such as: embassies, immigration offices for their respective procedures, educational centers and for applications required by the Civil Registry.

To request a Birth Certificate the person must:

  • Present personally with the identification document at the reception of Medical Records.
  • Complete the Medical Document Request Form (FO.RM-012), specify the date of birth and mother's full name.

*The process takes 8 working days and you will be informed by telephone to pick up the document.

In case of being requested by a third party, identification documents and written authorization of the patient must be provided, duly authenticated by a lawyer and complying with the requirements established by law in theGeneral Hospital Regulations of the Republic of Costa Rica in articles # 189, # 190, # 191.

Death Certificate

Death Certificate

The death certificate is made to certify the death of an individual. The certificates are requested for different purposes, by different institutions such as: embassies, banking institutions, life insurance companies, lawyers' offices for the completion of legal procedures and for applications required by the Civil Registry.

To request a Death Certificate the person must:

  • Present in person with the identification document (The interested party must have some degree of consanguinity with the deceased).
  • Make a written request.
  • Present a certificate from the Civil Registry showing the relationship (Birth Certificate or Marriage Certificate) to the Medical Records Reception.
  • Complete the Medical Documents Request Form (FO.RM-012).

Certificate of death declaration

Certificate of death declaration

It is the official document that declares the death of an individual within the Clínica Bíblica, which is signed by the interested party, the attending physician and the Auxiliary Registrar of the Civil Registry.

To receive the Certification of Declaration of Death, the person must:

  • Present himself/herself personally with the identification document (the interested party must have some degree of consanguinity with the deceased).
  • Verify that all the information contained in the Certificate of Declaration of Death is correct.
  • If the information on the Certificate of Death Declaration is correct, the interested party must sign the document so that it can be delivered to him/her.
  • The Medical Records Department sends the certificates to the Civil Registry on a weekly basis for the registration process.
