The birth certificate is issued to certify the birth of an individual. The certificates are requested for different purposes, by different institutions such as: embassies, immigration offices for their respective procedures, educational centers and for applications required by the Civil Registry.
To request a Birth Certificate the person must:
- Present personally with the identification document at the reception of Medical Records.
- Complete the Medical Document Request Form (FO.RM-012), specify the date of birth and mother's full name.

In case of being requested by a third party, identification documents and written authorization of the patient must be provided, duly authenticated by a lawyer and complying with the requirements established by law in theGeneral Hospital Regulations of the Republic of Costa Rica in articles # 189, # 190, # 191.
